What To Bring

  1. For New Patients: Completed New Patient Form [Download Here]

  2. Prescription glasses;

  3. OHIP Health Card;

  4. Up-to-date list of all medications and allergies; and

  5. Ontario Works / ODSP card (if applicable).

What To Expect

Expect a comprehensive, dilated eye exam to evaluate the medical health of your eyes. Each visit will be appoximately 90 minutes depending on the testing, diagnosis, and treatment provided in each visit.

  • Reduced near vision is a common symptom following dilation and is more bothersome for younger individuals.

  • Sensitivity to light is another common symptom following dilation. Consider bringing sunglasses.
