Preparing For Your Visit
No Show or Late Cancellation Fee: $75 without doctor’s note. We require 1 business day advance notice to clinic@ameyes.ca.
Allocate at least 90-120 minutes for your visit as diagnostic test and procedures may be provided same day for your visit.
Patients in Wheelchairs: Please inform our staff when making an appointment.
Refugee or Non-OHIP Patients: please call our office at 416-519-7404 to see if we can accommodate you and our pricing.
Requirements for Your Visit (Mandatory)
- (Must Bring - New Patients Only) Completed New Patient Form before your visit [Download Here] - please print and fill out before arriving at the clinic. 
- (Must Bring) Bring OHIP Health Card. 
- (Must Bring) Up-to-date list of all medications in a ziploc bag at EACH VISIT. 
- (Must Bring) List of allergies. 
- (Must Bring) Bring your corrective eyewear such as prescription glasses and contact lenses. 
- You will not be checked in if you arrive 30 minutes before or after your appointed time, as it will cause delays for other patients. 
- Patients are allowed to bring maximum 1 family member to their visit. 
Policies
- Patients are highly recommended to wear masks during their visit as doctors cannot physically distance during the eye examination. Masks are provided. 
- Disrespectful and violent behaviour and language will not be tolerated. 
- Patients must reschedule or cancel their appointments no later than 1 business day before their appointment. No show and Late Cancellation Fee is $75. 
Recommendations
- Arrive 15-25 minutes in advance of your appointed time to arrange for parking and checking in at the clinic. 
- Bring sunglasses to wear after your appointment as you may be dilated. 
- Arrange transportation after your visit as the effects of dilation may last for up to 6 hours. Additionally, eye procedures may be performed at your visit. 
- If you are diabetic, bring a snack to your visit to prevent your blood sugar from falling below normal. 
- Recommended to not wear makeup for appointments.
